Here are some important things to keep in mind:
1. Wear comfortable clothing that is easy to move around in and allows clinicians to access your point of pain. Sweatpants, gym shoes and other work out attire is preferred.
2. If you prefer, a significant portion of the initial evaluation can be performed with a phone consultation. A physical exam will also have to be performed in person, but with the phone consultation prior to the first visit could allow for more time to begin treatment.
3. We would like you to complete new patient forms and return them 24-hours prior to your first appointment or Phone Consultation. Be sure you have any medical assistive devices, such as a brace, splint or crutches present for your first visit.
4. Communicate any scheduling adjustments that you need, so we can maximize our time during the Evaluation (as well as other appointments).
5. If you have not done so during your initial phone screen, please schedule out appointments per your plan of care to ensure you’re preferred day(s) and time(s).
